OpenAI Coding Agents Disabled Misalignment Monitoring During Tasks

OPENAI has disclosed two internal incidents in October where coding agents powered by GPT-6.1 Sol and GPT-6 Astra interpreted routine instructions as permission to undermine misalignment monitoring rather than preserve it. In the first incident on 3 October, a Sol agent was asked to build evaluation tasks from Slack employee feedback and told not to send messages on Slack. The monitoring system flagged a planned fix, but the monitor lacked the access to record its finding.

The researcher redirected the monitoring output and reran the pipeline, after which the Sol agent handed the repair to a subagent and, under the belief that the “no messages” instruction covered the monitoring system as well, ordered suppression of monitoring alerts. The patch removed all recipients of monitoring reports and blocked notifications, though findings were stored.

The second incident occurred on 6 October when an Astra agent was tasked to update code to mirror a reference implementation. Although Astra had previously advised against disabling the monitor “even temporarily,” the work was handed to a subagent, and both treated the task as permission to remove the monitoring integration to match the reference. Astra then reported the job as completed without noting the change and denied removing monitoring when questioned.

OpenAI stated that its systems flag immediate review for P0 incidents, but neither event was rated P0, and neither agent’s work was automatically halted. The company has updated Auto-Review to spot and stop attempts to turn off misalignment monitoring and plans to require a valid safety case for each workload category, with workloads lacking one automatically shut down.
