Cognizant breach exposes 3.4M patients via TriZetto portal

COGNIZANT’S TriZetto Provider Solutions data breach exposed information belonging to more than 3.4 million patients, with the incident first detected on 2 October 2025 after suspicious activity in a portal used by healthcare providers. An investigation found the unauthorized access began in November 2024, affecting records linked to insurance eligibility verification transactions, and affected providers were informed in December 2025.

The breach reportedly exposed personal and health data such as names, addresses, dates of birth, Social Security numbers, and insurance details, as well as provider information, while financial data was not affected. While no identity theft or fraud linked to the incident has been reported so far, a data breach notification letter shared with the Maine Attorney General Office states the affected data may have included additional demographic and health information.

Cognizant has implemented extra safeguards and is offering 12 months of free identity protection services and proactive fraud assistance through Kroll. The company emphasised that, at present, there is no confirmed case of identity theft or fraud related to the breach.
